This document and FORM A are identified and authorized by the SPK "Declaration" (Section 13, "Restrictions Upon Use") and the "SPK Rules and Regulations Handbook." Both are available on the SPK owner website or at the office. "Standards & Procedures" adopted by the SPK Board govern key aspects of unit modifications and are also available on the SPK website.

An owner, or representative, is required to submit a completed FORM A, with any required documentation, and attain Association approval, prior to the start of any work.

A FORM A is a request for approval for a modification. Approval comes, in writing, from the Association Manager or the Board (dependent upon the type of alteration, modification, or replacement).

The completed FORM A, including all necessary supporting items and any other appropriate supportive information (manufacturer's specification sheet, drawings, photos, etc.), must be provided to the SPK Property Manager for consideration.

Submitted requests could be approved or subject to update, correction, or alteration to attain approval. The Association Manager, depending upon the type of alteration, could provide immediate approval or could refer the matter to the Board or Rules Committee for review prior to a decision.

The SPK goal is to respond to FORM A submissions within 7 days, but the process could take up to 30 days. All approvals are contingent upon compliance with SPK Rules & Regulations, applicable Standards and Procedures, Association By-Laws, and the SPK Declaration, as amended from time to time. The owner accepts responsibility for any damage, accidents, project clean-up, or other issues caused by their work.

  2. If the owner chooses a product other than the suggested doors, product specification information must accompany this FORM A for approval. Addendum — Standard & Procedure #3: Exterior Sliding Glass Door Requirements

This procedure applies only to new or complete replacement of sliding glass doors — not to minor repairs or adjustments to existing sliders. Please see the Association Manager for the most up-to-date specifications and authorization.

Requirements: Slider product must meet Florida's most stringent code requirements for "Hurricane Force Winds and Flying Debris." All new installations/replacements must meet current State of Florida and Charlotte County building codes. If E-coated glass is installed, the glass must be tinted bronze because this glass has a green hue.

Notes:

  1. The owner must provide the SPK installation standard (Appendix A, below) to the contractor prior to pricing and contract agreements.
  2. Prior to commencement of installation, a completed FORM A "Request for Sandpiper Key Unit Alteration/Remodeling" must be submitted to, and approved by, the Association Manager.
  3. The contractor selected must be licensed and insured in Charlotte County, Florida. The contractor shall obtain a permit from the Charlotte County Building Department and the owner must provide a copy of that permit to the Association Manager prior to commencement of work; a signed final permit must also be provided to the Association Manager once work is completed and approved by the County.
  4. As part of this installation, waterproofing of the lanai under the slider track to prevent damage into adjacent or lower units is required (see Appendix A installation guidelines below).
  5. The undersigned owner understands compliance with all of the above requirements is the owner's sole responsibility, and failure to comply or provide any required documentation means SPK approval will be denied and removal of the sliding glass door will be required until all necessary steps have been met.

Appendix A — New Sliding Glass Door Installation (SPK Recommended Guidelines)

Preliminary Notes Use extreme caution when measuring the vertical dimension — no concrete cutting is permitted. Allow 5/16" for peel-and-stick and .024 flashing pan for frame height. The flashing pan must be one piece from the back of the bottom track to the end of the concrete slab, bent over the end of the slab a minimum of 1"; a splice is expected since break-work maximum length is 10'. Where a railing/screen frame sits tight on the flashing, drill 3/8" holes every two feet to allow water to escape.
Prepare Opening After removal of the old frame, inspect all concrete areas and repair any damage. In 2-bedroom units, before installing new framing, open the master bedroom closet wall and install additional support to secure the side rail of the door frame to meet current hurricane/building code wind-load requirements; vertical flashing is required at the corner of the stack/closet wall for waterproofing.
  1. Install 2 layers of 6"-wide peel-and-stick so the screw holes in the bottom track are a minimum of 1-1/2" from the tape edge (use multiple widths if needed) — this seals the bottom track screw penetrations.
  2. Fashion the flashing pan from break work using two ten-foot lengths for maximum overlap. Install NPC Solar Seal caulk along all edges and every 3" inside the lap.
  3. Install NPC on the sides of the pan and set the flashing pan in place; if needed, fasten each side at the tops of the side flaps (not the bottom of the pan).
  4. Install two more layers of peel-and-stick tape so the bottom track holes are at least 1" from the tape edges; do not let tape stick out past the bottom track.
  5. Use stainless steel tapcons in the bottom track (shortest length per engineering). Drill holes in the concrete slab and install tapcons with appropriately sized O-rings; snug to put pressure on the O-rings without over-tightening (replace any O-rings that are over-tightened).
  6. Install NPC if needed at the top edge of the pan sides that stick out past the slider side tracks.
  7. Photos of the steps above are required as proof of compliance and must be submitted to SPK.

Addendum — Standard & Procedure #7: Replacement of All New Windows (Buildings 1–8)

Requirements: Regardless of vendor or window type selected, these specifications are mandatory. If E-coated glass is installed, the glass must be tinted bronze because this glass has a green hue. The unit owner assumes full responsibility for any damage that might occur to common elements.

Contractor shall obtain a permit from Charlotte County, as required, and submit a copy to the Association Manager prior to commencing work, along with current copies of Charlotte County license, Workers' Compensation, and Liability Insurance.   2. It is the owner's responsibility to select a company whose windows and installation meet the "Approved Materials List" available in the Manager's Office. Permits, as required, must be secured before work begins.

If the owner selects a window other than that specified, they must provide all specification information to the Association Manager's Office for review by the appropriate Committee(s) and approval by the Board of Directors prior to purchase and installation. Failure to secure such approval may result in requiring removal of a non-approved installation.
  4. Prior authorization for a replacement or installation must be obtained using this FORM A, "Request for SPK Unit Alteration/Remodeling," available in the Manager's Office.

Addendum — Standard & Procedure #6: Solar Film Requirements

Addendum — Standard & Procedure #8: Hurricane Shutter Specifications

Requirements: Shutter design and specifications must meet all criteria in the most current State of Florida Building Code (Class A Hurricane Protection) and "Coastal Zone Wind Load" requirements. Sill anchors must be power caulked-in with stainless steel bolts and neoprene washers, sealed with Vulkem sealant, and all materials must be permanent corrosion resistant. The owner will have the installer and/or supplier sign a waiver of lien absolving Sandpiper Key Condominium Association, Inc. of any financial responsibility resulting from non-payment of material or labor by the unit owner. This waiver applies only when shutters are installed on common elements.
